@jlsalinas7 That actually bothers me less. E. faecalis is increasingly recognized as a common cause of IE (eg https://t.co/HfgC2qXE6s) when found in blood cultures, as are certain strep species. But there are some, uh, *interesting* recs in this document

2/ E. faecalis is likely underrated as a cause of infective endocarditis (IE). Dahl et al found endocarditis in ~26% of 344 pts with E. faecalis BSI who had an echo. This prevalence is similar to that of Staph aureus BSI! Watch out for E. faecalis BSI! h

E. Faecalis bacteremia has higher than expected incidence of IE. ⬆️ in community acquired, immunosuppression but ⬇️ in nosocomial. What about neutropenic bacteremia? Dr. Greene used to say IE low as we blast with ABs fast with neutropenic fever patients.

Don't ignore those pesky E faecalis + Bcx cases w/ high risk clinical features (defined here as community acquired, unknown origin, prosthetic valve, immunosuppression, monomicrobial, >= 3 Bcx bottles)... IE is lurking... thx @north_noelck! #endocardit

I always worry about enterococcus but the 26% here still surprised me, as did the even more pathetic than expected performance of TTE- thanks!
